There are many components to the information sheet that contain pertinent information for the physician and staff of the medical office. Therefore, there are many reasons why these sheets are so important. However, not all information sheets are the same and some may contain different information based on or according to the type of practice. One of the first components on these sheets is the patient address and telephone information. It is important to have this information and make sure that it is kept up-to-date so that if the office needs to get a hold of you for any reason, such as: being scheduled for a test, then they have all of the correct information to do so. The next important component is insurance information. It is also important to keep this information up-to-date so that the facility can properly bill the right insurance, if you have insurance, and process the correct co-pay amount if you have one. If you should happen to change jobs and your health insurance company or coverage changes this should be changed on your form on your next visit. Also in this area of the sheet, there is a section where you will record secondary insurance coverage if you have a spouse or parent that may still have coverage for you. Again, this will be extremely helpful to the billing staff of the facility. Some information sheets will also include a medical history of some sort. Some are very extensive and want to know absolutely everything. I received two separate information â€Å"packets† from two different practices. Neither one of them is very extensive in what information they want for your medical history, listing only a few different things. No where on either sheet could I find a place to write any kind of allergies that you may have. In my own opinion, I believe this to be an extremely important thing to have. I do know from experience that the Delton Medical Center has this information in their computer, but if by chance something happens to their computer system and the information gets â€Å"lost†, this information would be a good thing to have in the chart also. Some facilities that I have been to, such as a dentist office, go into an extensive medical history sometimes including your immediate family such as: your parents, grandparents, and siblings. By going into such depth, they can use this as a reference or â€Å"warning sign† for illnesses or conditions that may arise for you as the patient. Some medical practices will also include the financial policies and a form stating what the responsibilities of the physicians and staff are and what you can expect from them. Also what they expect from you as a patient of their practice. Many will include when payment is expected and the cancellation policy. In almost all cases there will also be â€Å"consent to release† medical information so that your information can be released to insurance companies and possibly employers. If you should decide to change offices or physicians, this form will also be used. Lastly, in this packet you may receive, there is a HIPAA Privacy Authorization Form. This also is an important document for both the patient and the medical office. This form will give the physician authorization to disclose health information pertaining to you if need be. If you do not fill out this form then the physician or medical staff cannot release any information to anyone about your condition as your health condition is confidential. I think of it as a type of a permission slip for someone to obtain your information but only if you have stated this on the form. As I mentioned earlier, not all information sheets are the same. They do not all contain the exact information on them. Some of them are short and sweet and some of them take you a long time to fill out. Some will take your information using a computer so that you don’t have to fill out all the papers. Most hospitals have switched to using this computer method. Someone will come around with a laptop computer on a cart complete with a scanner and ask you the questions that they believe to be important such as: our name, phone number, address, and emergency contact. At this time, they will also scan your insurance card if you have one so that it can be attached to your file in the computer record. Whether it is long, short, computerized or hand written, all information on a patient information form has its importance for both you and the medical facility. There are many different types of sources like sales, loans, and investors. Each has different terms, benefits and disadvantages. Business owners tend to use two or more different sources in order to fund their business. Business finance sources fall into two main categories:- Internal funding Internal funding comes from the profits made by the business by sale of products or assets. These are the funds which the company has on its own like the income of the company, the money they have of their own etc. External funding External funding comes from lenders and investors. The most common external finance sources are loans. Short and long-term loans require borrowers to repay funds at an interest rate for a set period of time. Overdraft loans allow a borrower to spend a certain amount of money, and the lender charges interest on the overdraft amount. Before deciding which method is best for a company, business owners should consider a variety of factors. The cost of the business finance source usually is the most important factor considered. Owners look at the interest rates and payment plans to determine the profitability of obtaining a certain funding source. Businesses that have a history financial stability may want to consider an internal source of revenue before opting for an external source. Its also important to determine how long the business will need additional funding. A short-term loan would be best for projects that would only take a short time to complete. Business finance start-up generally refers to the cost to start a new business. It includes determining, calculating, and obtaining start-up costs, as well as managing those finances effectively to ensure the profitability of a new business. The first steps to business finance start-up are to determine and estimate the amount of funds needed to open a business. These start-up expenses may include one-time fees, such as permits and licenses needed to operate the business. Initial costs may also include on-going fees, such as rent and utility payments. Business owners usually only include the necessary expenses when determining the total cost to start-up. As a sole trader or other non-limited business, personal assets can be at risk in the event of a failure of the business, but this is not the case for a limited company. As long as the business is operated legally and within the terms of the Companies Act, directors or shareholders personal assets are not at risk in the event of a winding up or receivership. And as often happens on occasion, such events are not always under our own control. There is no obligation for a limited company to commence trading within any set time period after its incorporation. Creditors can claim all your property to satisfy debts, and if this is insufficient, you may be declared bankrupt. An undercharged bankrupt is forbidden to start another business or to become a director of a limited company. Separate Entity Due to its very nature, a limited company is deemed to be a separate legal entity from its owners. This has several advantages, including the fact that the company will exist beyond the life of its members. If they retire or die, the company will continue to exist and operate. This ensures security for employees and other members and also is an advantage which other legal forms of business are not subject to. Ownership and Control In the case of Private Limited Companies, the Directors are also usually the main shareholders of the Company. Thus both the ownership and control of the business remain in their hands. Decisions can be made quickly and easily, with little fuss, allowing for a more successful business management platform. Company Name Part of registering a Limited Company, includes the registration of a Company name. This name will help identify the business in the marketplace, separating it from other Companies and protecting it. 1.8 A sole trader is a business form that allows one person to be solely responsible for the financial transactions of the business. The benefits and disadvantages of this responsibility are numerous and should be weighed carefully. Another term for a sole trader is a sole proprietor. Precisely, it refers to the person responsible for the daily organization of the firm and for its profits and losses. Being a sole proprietor as it in legally known in the U.S.is a benefit to many people who are looking to start their own business. It is one of the most common types of businesses that can be formed and involves only one person as the responsible entity of the businessholding that person completely accountable for any debt or liability that the business might incur. Although sole traders have many advantages, they also have many significant disadvantages. Liability The main disadvantage to being a sole trader is the liability that the business owner yields. Being held responsible for any lawsuits or potential damages is not only dangerous to the sole traders business, but it can be detrimental to her personal life as well. Unlike modern business corporationssuch as LLCswhich allow the business to be a separate entity, preventing anyone from holding your personal assets responsible for your business, sole traders are personally responsible for their business. Responsibility One of the main disadvantages to many people for running a sole proprietorship is the complete responsibility that the sole trader has. Completely variant from business to business, since each business has its own type of operations, sharing responsibility takes a huge burden off of most business owners. That is one reason for the popularity of Limited Liability Corporations, Limited Liability Partnerships, and partnerships. These businesses each allow some owners to share or take less responsibility, leaving them to grow and improve their businesses more thoroughly than if they did not have complete responsibility. Lack of Investors When it comes to being a sole trader, the business owner can have a difficult time for growing. Not only because of the lack of time that she has because of her tremendous responsibility, but due to investors lack of interest in a sole proprietorship. Companies are more apt to invest in corporations that have the potential to expand. They also prefer the other benefits of corporations, such as their legal structure and lack of personal accountability. Lack of investors can mean lack of growth for many companieswhich can leave many sole traders running a stagnant business.

Roy Wilkins and the NAACP: A Life Dedicated to the Civil Rights Movemen

The life of Roy Wilkins is a story of one of the greatest civil rights leaders the United States has ever known. He was an underdog that came from poor beginnings to become a leader of the NAACP, for twenty-two years. A true example of what someone can do if they put their minds to it, no matter what color they are. To begin the journey through Roy Wilkins life, we will start with a little biographical information. Roy was born in St. Louis, Mo. On August 30, 1901, as the grandson of a slave. His mother died when he was three years old, so he and his sister were sent to live with their Aunt and Uncle in St. Paul, Mn. There they raised him in a low-income, integrated community. Although he was poor, he did attend integrated public schools in the city. After graduating high school, Roy worked his way through the University of Minnesota, where he majored in sociology and minored in journalism. He had various jobs to put himself through college. He worked as a redcap (a baggage porter), waiter, stockyard laborer, and a night editor. While in college he worked as the night editor (to help pay his way through) of the Minnesota Daily, the school paper and a black weekly, the St. Paul Appeal. After working all these odd jobs he managed to put himself through college. After graduation, he took a position as a journalist for the Kansas City Call, a black weekly paper. He stayed there for seven years, acting as managing editor from 1923 until 1931. Although the job at the Call was good, he left it in 1931 to join the NAACP as Assistant Executive Secretary, under Walter White, who was Executive Secretary at the time. In his new job, his first assignment was to investigate discrimination on a federally funded flood project in Mississippi, in 1932. Due to his findings of discrimination at that project, he was successful in getting Congress to take action to stop its practices there. After a couple of years as Whites assistant, in 1934 Wilkins seceded W. E. B. DuBois as editor of the NAACPï€ ½s magazine, the Crisis. In that same year, he suffered the first arrest of his civil rights career.
